Description | This is a measure of quality of life or health status. A proxy version is available and should be used for children aged 4-7. The instrument consists of an item for each dimension, followed by a visual analogue, in which participants must rate their health on a scale from 0-100. An adult version is also available and appropriate for older adolescents. |
